1. Responsible party & Information Officer
MY-LO is a private body as defined in PAIA s1. Our Information Officer is the contact point for PAIA requests:

- Best address for requests: the above email, marked "PAIA request".
2. How to request access (Forms A & B)
- Form A — request access to your own personal information held by MY-LO.
- Form B — request access to other records held by MY-LO.
Send your request to the Information Officer with enough detail to identify the record. We will acknowledge and process it within the periods PAIA prescribes (30 days, extendable by a further 30 days where the request is complex or voluminous, with notice).
3. Categories of records held
- Website enquiries — name, business, email, message, sector, consent flag, timestamp (from the contact form).
- Lead records — sourced/referral leads (company, sector, contact, email, phone) used to operate the referral programme.
- Referrer records — referrer account, code, and commission earned.
- Client engagement records — signed client agreements, quotes, and configuration notes (held as processor where the client is responsible party).
- Internal PAIA/POPIA request logs — a record of requests received and our response.
4. Categories of data subjects
Website visitors, enquiry submitters, leads/prospects, referrers, and clients (including their staff and end customers whose data the client processes).
5. Fees
Where PAIA allows a fee, we charge the prescribed fee set by the Information Regulator. Routine access to your own personal information is free. We will confirm any fee in writing before processing a request that attracts one.
6. Internal appeal & remedies
If we refuse a request, we will give written reasons and tell you of your right to internal appeal (PAIA s74) to the Information Officer, and your right to approach the Information Regulator:

7. Records relating to PAIA requests
We keep a record of PAIA requests received and our responses for the period required to demonstrate compliance, then delete or anonymise them.
